At Open House Center, our team advocates for both health and independence. Our care managers serve as trusted intermediaries between clients and carefully matched care specialists who are deeply committed to helping individuals maintain their independence for as long as possible. Each care plan is thoughtfully developed by licensed, certified, and experienced nurses and educators, addressing a range of needs such as social isolation, medication adherence, health maintenance, and overall wellness, even when away from home.
We offer support with:
Comprehensive assessments
Communication with family, friends, and support networks
Coordination of healthcare and community services
Home safety evaluations and wellness planning
Medication management
In-person assessments with a licensed nurse and a multidisciplinary team who will take the time to get to know the client

Our day program provides clients with a vibrant and supportive environment filled with meaningful activities and joyful moments. Through recreational activities like gardening, games, cooking, and arts & crafts, clients are encouraged to express themselves, build skills, and connect with others.
We also host engaging monthly events featuring guest speakers who share topics of interest to our clients. Each year, we come together for exciting community traditions like our Olympic Games and National Autism Acceptance Month parades—celebrating inclusion, ability, and the unique strengths of every individual we serve.
Whether it’s Halloween, a client’s birthday, or just another Friday, Open House Center believes in making every day special. Our parties are more than just decorations and treats—they’re moments of connection, joy and community. From costume parades and themed games to dancing, music, and delicious food, we love creating inclusive celebrations that bring smiles to our clients’ faces and strengthen our bonds as a family. At Open House Center, every occasion is a chance to honor individuality, spark laughter, and celebrate the beautiful lives we’re privileged to be part of.
One of the pillars of our mission is to foster independence and meaningful community engagement for our clients. Each month, we host Community Outings, where clients venture out to local businesses, parks, and events to connect with the world around them in fun and enriching ways.
What sets OHC apart is our commitment to ensuring that every client receives the care they need—wherever they are. Whether onsite or out in the community, we provide transportation, dedicated caregivers, and nursing support when needed, so that all participants can enjoy these experiences safely and comfortably.
Open House Center strives to be a welcoming and dependable source of education and advocacy. Our goal is to become a vital part of the Concord and Contra Costa County reflected in our community outreach initiatives.
Below is a short list of projects Open House Center has undertook in the past couple of years!
Open House Center was at the forefront of supporting Contra Costa County at the start of the COVID-19 pandemic when it opened its doors as a vaccination site. In collaboration with the Contra Costa Health Department of Public Health and Regional Center of the East Bay, Open House Center administered vaccines to residents over the age of 65 and other healthcare workers. Our administrative staff assisted in getting members of the community to register with our clinic and ensured the drive-up experience would take no more than 20 minutes per individual.
Open House Center has been organizing annual door-to-door holiday carols since 2019. Our staff members prepare a diverse set of songs and dance to celebrate all holidays in the month of December.
